Team Entry Form

We are pleased to inform you that we will continue to use the online participation system. As a coach, you can register or log in to your existing account to submit your team information.

Important items to note:

• Please be advised that any contestant winning a monetary award will be required to furnish his/her full legal name and Social Security Number on IRS Form W-9 prior to receipt of the award. The forms will be distributed to winners and MUST be completed onsite.
• Team information must be finalized by April 8th, 2024. Changes in designation of team members, alternates and graduate students may be made up until 5:00 pm on the day before the contest day, April 16th, 2024.
